I just created a blank WordPress installation to test this.
I uploaded an image under Media > Library and then clicked on an image. All ISC fields showed up. See the screenshot.
Is this where you are missing the fields?
Do you see any script errors?

I am not missing fields, but asterixes at fields, that are required. If no fields are required, then there is no need for the frase »Required fields are marked *«. If any fields are required, those should have an asterix.

And thanks for making me aware of something most users wouldn’t bother with!
Would you wish that some of the fields were required? I know that was the case at some point, but have to look back a couple of years to see if I had an intention of disabling that behavior.

Well, in order for this to solve its purpose, I think some fields should be required. The important question is, which fields should be mandatory to fullfill the requirements of copyright protection? Those fields then should be marked with an asterix.

I finally found the time to investigate this. I first wondered, where did I remove that required-icon.
It turns out that the “Required fields are marked” message is not included in ISC. It is part of WordPress core and added automatically when the media options are extended by other fields. I believe this is an oversight in core and could be fixed there. Nothing I can do except of writing a ticket for the core team.
