# Embedded contact form

 * I want to embed a contact form into the Thank You page. I’ve tested the form 
   on a regular “page” and it works. But when I paste the form code into the thank
   you page, it doesn’t work.
 * I’ve tried inserting it in the “simple text” block as well as the “html” block.
 * Both blocks DO display the form and submit button; however, when you fill out
   the form and click ‘submit,’ none of the information goes through and basically
   the form doesn’t work.
 * Any ideas on a workaround?

 * The shortcode should execute if you are using HTML Blocks ( they won’t in text
   block as they are not WYSWYG editor)
 * Can you tell us which plugin is generating that shortcode?
 *  And we can try and replicate at our end of what you are trying to do.

 * I created the form in a standard page editor, then copy/pasted the shortcode 
   into your Thank You page. It looks like this:
