I want a contact form for several locations on my site, they are all located to the same email address, but they need to have diffrent fields (to fill out), I need about 3 diffrent ones, I can’t find anythinng, is it even possible?

    Thank you in advance.

  • As far as I know yes…there is also a plugin you can use.

Wp-Contact Form- Ryan Duff, Firas Durri
WP Contact Form is a drop in form for users to contact you. It can be implemented on a page or a post. It currently works with WordPress 1.5+

And theres ways to add these forms via any page or whatever just using the contact form call tag and there’s a way to add the extra forms via the Create Page section of Control Panel. But someone else might be able to tell ya more about that, because I never really needed to add more fields…hopes this helps.

< goes back to drinking beer >… =) mmm.. good

I have that installed actually, I can’t seem to make it work though. I can’t fiind where to edit the fields and i dont think you can use it twice with two diffrent set of fields :S

Try on the page you’re editing…..scroll down to this:

Add a new custom field to this post:

Then add a new (key, value or action) for the new field once. Also, I think when ever adding this to a page: < ! – – contact form – – > without any spaces, should use the same email address for all. When editing the fields of the Contact Form itself, try going to the Plugins section of Control Panel, then Plugin Editor, then find the WP-Contact Form plugin and try editing that file once. I’m not sure though, because I never attempted to try it. Moshu is around and I’m sure he can help ya better…

Still can’t get it to work :\

Audiobreak. Look at this and tell me if this is what you are looking for.

If so yes indeed it will work just there is a little bit of info you need. I can email all the html files to you and tell you how to use them.
