 * There are many quiz plugins that you can customize to look like the quiz you 
   shared.
    I recommend “[Quiz Maker](https://wordpress.org/plugins/quiz-maker/)”(
   but you can use the plugin you want), you can create your own quiz and your questions
   for free, choose your type (checkbox, radio, date, etc.), and you can set the
   approval percentage and the personalized message if you pass the questionnaire,
   the same option if you fail. You can also see the results and the users who did
   it. It has many settings to customize, check it out and tell me if it works for
   you.
