# Edit Contact Form

 * I am using the “Unique” theme on my website: [http://www.dublinprocessserver.com](http://www.dublinprocessserver.com)
 * I would like to add a contact form through this theme, but the included form 
   does not have all of the fields that I would like.
 * Can anyone tell me how to edit the form to include additional fields?
