 * I’m looking for a plugin (or if necessary, hack) that can function as an email
   contact form with a twist: I need to dynamically generate fields based on a wp_query.
   For example, each post in a custom post type translates as a checkbox on the 
   form. Are there any solutions out there for this sort of thing?
