    I'm new to wordpress. So please bear with me.
    I need to create a page which accepts some inputs from the website visitor and submits the form. The data is then validated using javascript and then stored to db using php and finally an acknowledgement page is shown to the user.
    I'm not clear as to what should be the locations of storing these files(html,css,js and javascript) and how to link them.
    Any examples or pointers will be of great help.

    I would suggest using a plugin that creates the form for you, rather than trying to do it all from scratch. But it does depend on what type of JavaScript Validation you are wanting to do.

    One of the most popular Form plugins for WordPress is: http://www.gravityforms.com/

    If you are looking for a free plugin you can try: http://wordpress.org/extend/plugins/contact-form-7/ There is an add on plugin that will let it store to your database.

    An alternative that I personallly like is: http://wufoo.com/ You would just embed the form on your site with the embed code, but the results are stored at Wufoo.

    Hopefully this gives you a starting point to consider.

    A new alternative which I've found is JotForm. They've also came up with a WordPress plugin that you can easily use.
