Sounds like an incorrectly coded content filter. Have a look at this guide and let me know if un-checking the content filter option fixes your issue — if it does, you could contact your theme author to have them fix their content filter. 😉

The Apply WordPress Content Filters has never been checked… Is there any other reason why it might be having this problem?
Ok, here’s an easy test. Disable NGFB and select a featured image — does it create duplicate images in your content? If so, then you should contact your theme author. If there are no duplicates, but enabling NFGB (and clearing its cache) creates the duplicate images, then it’s probably a filter issue. Normally this would be from a content filter, but as you reported, that option is disabled on the Advanced settings page. So, if enabling NGFB creates duplicates (and not your theme’s design), then provide a URL to an example post/page showing the duplicate images, and I’ll have a look. Perhaps the CSS around the duplicate images can give us a clue as to where they are coming from…

Ok so I think there is a duplicate with it enable or disabled. The only weird thing is it didn’t always do this which is why I thought it might have been this plugin… oh joy… Thanks for being helpful. Any other recommendations?
Did this issue appear after updating your theme?
You could try disabling plugins — especially new ones — and see if that helps. If you use a caching plugin, be sure to flush it’s cache after disabling a plugin, just to make sure you have accurate page content. 😉

Did you install any other Facebook related plugin recently? Some still use the old V1 API, which is not compatible with the latest Facebook API.
