Drop Down Product List – Product Page Purchasing

I need help and ideas on how to do this. I want to create a product page purchasing form or order form. On this order form I want to have a drop down of the list of categories which are select states. Once they pick out the state from the categories drop down menu. Below that will be another drop down list which will show all the products along with cost on one line.

I want the user to select muliple products. Once finished they will then select a quantity of years they want the product and actual quantity it self and put all the products into a shopping cart below.

I have woocommerce along with all the products set up already.

Now my question to all of you is… Is there any plugin that will work with this idea? What is the plugin and how would you edit it???
