The problem is more complex in this case:
– when in the backend, you don’t know the language somebody prefers, so when sending a mail it will be done in your language (will still be a problem).
– when creating the form, the value of the dropdown field (and any other field) is the same as the label. In this case you want to change the label, but not it’s value, otherwise if you edit a booking (where the person was in another language than yours), I wouldn’t be able to tell which value the person choose (because the values in your language would be different than the values in the language of the booker). So you would want an extra “label” part for multivalues.
But it won’t solve the mail problem, since wordpress is not multi-lingual by itself, and if not logged-in users (or even a logged in user) change language by clicking on a flag, I don’t know about it when an admin is in the backend. I would need to keep the language the user prefers as extra booking info, which would then need to be used when showing values when editing and also when sending mails.

if I’ve understood you well, then my problem could be solved by adding a language selection field (for example a drow-down-field with the values “German || Italiano”)?
Can you give me a short example how I would have to proceed for the confirming E-Mail in this case?
Is something like
If language-selection-field = “Deutsch” then
German Text
End if
If language-selection-field = “Italiano” then
Italian Text
End if
possible?

I added code that takes the current language of the user and/or booking into account, so mails will be translated to their language.
However translation of the dropdown part is not yet done.

A short explanation for other people, working with the multilingual plugin qtranslate:
Under EME – settings – mail there may be defined various standard mails (confirming booking, booking cancled…)
It hasn’t been possible to consider the language of a booking person so far.
Now, this is possible.
Mails can now be defined in the respective languages.
Example for German and Italian:
German version:

German Text, for example: Ihre Buchung wurde registriert…
<!–:–>
Italian version:
<!–:it–>
Italian Text, for example: Confermiamo l’iscrizione…

I would be the ideal case if in the Italian version the links directs to the Italian version of the event, but it always directs to the event in the default language (German in my case).
Can you think of a solution to this?
