    I’m looking for a plug-in or widget that can give me a contact form 
   that drops down from a button, such as the “Request an Appointment” button on
   this site: [http://www.columbianeurosurgery.org/](http://www.columbianeurosurgery.org/)
   Which is a WordPress site. Thanks, Andrew

 * Many thanks. I’ll check out their CSS file. BTW: Do you by any chance know a 
   good link where I can read about CSS sliderboxes and sliders in general? I’m 
   a bit of a newbie.
