Does premium version would let me do this?

Something like that is doable with the Pro version, but there isn’t a built-in widget or shortcode for it. Basically you could create a custom form and integrate the functionality via your own custom shortcode, which would be included in the form. Any submitted information would then be attached to the submitted post and then also whatever your custom code is designed to do.
