Does not show To, From, Message, fields on mobile – and so can’t add to cart

The “To”, “From” and “Message” fields are working on desktop but do not show at all on mobile. As a result customers cannot “add to cart” on mobile as they get the pop-up error: “Please select some product options before adding this product to your cart.”
Please help! We have checked the box “Apply fix for missing fields” and it does not fix it. I set WP_DEBUG to “true” in wp-config and got no errors.
Running latest update.
