Just install more than one contact form. One of each page. Have a look at the contact form 7 plugin. You can create as many forms as you want.

A plugin would be the easiest way if you’re not familiar with PHP and HTML etc.

    Contact Form 7 is a good’n.

Thanks guys. How do you keep the plugin from appearing on every side widget?
Anyone have a link that explains?

Why on earth would it do that? That’s not what plugins do.

Have a look at the plugin we recommended

Thanks. I will read more before I ask more.

I prob phrased that wrong. The contact form appears on every page in the same widget area.
I want the contact form in same widget…I just want it to have different contacts on each specific page.

Ok that is different. It would be easier to have the forms in the actual page and not in the widgets. You’ll need a different plugin then. Maybe something like this – http://wordpress.org/extend/plugins/wp-widget-changer/
    (I haven’t tried it.. )
