Different comment fields/styles on different pages?

  • I am trying to figure out if there is a way to display different comment fields/forms/styles on a per-post basis.

    So, if I write a page and want to use comments as a survey, like this:

    Name:
    Email:
    University:
    Major:
    Height:

    I’d want to add custom fields to the comment form/display, but only on this page—how can I do this?

    One option I can think of would be to use a custom CSS per-post plugin and set all undesired fields/elements to display:none. But I’d think there would be a cleaner way to do this.

    How about a plug-in like Custom Comment Fields. I found this by searching on the keywords custom comments in the plug-in repository.

    I saw that but it still seems to be more than I need. Custom Comment Fields works on the level of post types, so I would have to create a separate post type every time I wanted to get customized comments.
