Hi! Thank you for plugin.
I have error in console for some reason.
Uncaught ReferenceError: datepickerLang is not defined

Cache and optimize plugins is deactivated.
How can I fix it? Thanks.

Sorry to hear you are having this issue.

I see the jQuery is loading on your website.

Can you export the form and share it using Google drive and we can test the form in a vanilla installation?

In case it works the issue could be a plugin or theme conflict.
