 * I am looking for a free template that would allow the user to enter fields for
   the church they attend, after approved, I would like other users to find churches
   by State/City in a combo box. Can this be done esily and are there any available
   templates to accomplish this? How else can it be done?

 * To make it more clear, i would like the user to enter the necessary data in aboout
   15 different field boxes (name, adress, phone …). Then after another user uses
   a combo box, they shuld be able to find a church by tth statte and city. After
   selected from the combo, all the fields will be displayed in their proper field
   boxes.
