 * Hello.
    The ability to add new form fields, or change the structure of booking
   form is available in paid versions of plugin. You can test it in live demos at
   this page. In the free version of Booking Calendar you can change the labels 
   of the form fields and activate/deactivate active form fields. Kind regards.
