 * Hello, I’m fairly new to this, but I’m trying this plugin out on a temporary 
   page I’ve set up on my site currently, I’m trying to find out how to customize
   the fields for the form that are displayed by default?

 * Hi I’m also interested in this. The main thing is adding an email address and
   birthdate to the form. How do I accomplish adding custom fields?

    You need to change
   from code level for add additional fields. I am planning to make dynamic so that
   everyone customize form fields from wp admin.
