Custom Visual Editor Tintmce button

Hello again,
I have made some custom buttons, but i really would love to do is add a button which if i click a window popsup and the user can type a simple text value, then when submited it will send the text into html content area and but wrapped in a simple container like a div.
for example if they type: MATTHEW
it will insert that as html like this: {{xmp.r[‘MATTHEW’]}}

I already have the function inserting the code into html however i cannot ask for a text value and insert that anywhere i would like.
