 * Is there a way to add a custom visual editor button WITHOUT the help of a 3rd
   party plugin?
 * I’ve been looking for a way to do this forever, but can’t seem to find the answer.

 * To do so properly, you need some kind of plugin, but it could be your own custom
   creation instead of a 3rd party product. For starters, review [TinyMCE Custom Buttons](http://codex.wordpress.org/TinyMCE_Custom_Buttons).
