Custom Theme (Post Form not displaying)

I have created a child theme based on twenty twelve.

At the top of my post there is a ‘Leave a reply’ link.

When I click on the link it goes to a url with #respond, but no form is displayed under the post to enable someone to leave a reply.

The problematic page is located at this url:

Any advice would be very much appreciated.
