Custom short url with custom content

    Hello,
    Im try at google to find answer for my question but cant, what im try?
    Im try create simple short url with custom content

    Example if my fill url is
    http://wordpress.org/support/forum/how-to-and-troubleshooting/

    i need create short
    http://wordpress.org/short/256

    Where is /256/ its a post ID

    But now this short url must point to custom page where i will have for example only title and custom field content…. so no header no footer and etc…
