Custom login url is not working

    Our custom login url is not working, please can you assist to troubleshoot?

    Also:
    The tracking script is fine, but it needs a form code to hook into on the front end. This form code would be basic html that you would paste on your page that would enable the form to display on your site. This field is blank on their site and we need this.
