# Custom HTML form – doesnt pass checkbox data

 * Hi,
 * On my custom form I’ve got a several checkboxes showing services:
 * `<div><input class="ccf-tooltip-field" title="If you would like to book an additional
   service please select here." name="additional_services[3]" type="checkbox" value
   ="Gold Service" /> <label class="select" for="additional_services">Gold Service
   </label></div>`
 * however even if this is checked the email received doesn’t show the data…
 * Any ideas?
