Sounds like I need some more information… If you enable debugging (Settings → Post Thumbnail Editor) there should be some more information printed to the javascript console. (ctrl+shift+j in chrome). Do you see anything there? (Slightly better instructions can be found here: http://sewpafly.github.io/post-thumbnail-editor/editor/#toc_12)
Another thing you can try is downloading the latest version from github().
To help me in reproducing, what browser(and version) are you using. What wordpress version?
I’m also getting this issue.
It happens in all common browsers (Firefox, IE, Chrome, Safari) and I’m using the latest versions.
I’ve used your plugin on other websites and they are still working fine, however this is a new build. I have also downloaded an older version of the plugin (from another working website) and that didn’t work either.
I’m using WordPress 3.7.1.
@wordchris – are you seeing any information in the debug logs? What is the last entry in the network tab?
Thanks for your quick response.
I’m having the issue in FF v 25, Chrome v 31 and Safari v 5. I’m in ie 10 and the thumbnail menu isn’t even showing up there, but I don’t really know if that’s ever worked. We’re in WP 3.6.1.
Here’s the javascript console data:

event.returnValue is deprecated. Please use the standard event.preventDefault() instead.
The first error seems to be coming from wordpress, the other errors don’t appear to be PTE related either… Have you tried disabling all other plugins?
When you use Chrome, open the web debugger to the network tab, then click the Crop button. What does the request to admin-ajax.php look like? What is the response?
