Critical error using a form

Hello,

I´m using this plugin with two forms on this site: http://www.novadentalcr.com but recently it shows an issue. I cannot access the PDF forms. I just received a critical error message with an email with this information:

I tried to fix this error with no success. I hope you can help me to fix it or find a solution.
