creating page with plugin wordpress

Hello every body,

I have developped my own plugin to create a form to add customer and into this form i need to create an other form to add a responsible’s informations, the first form work correctly but i need to create the second form into an other page.php but this later appear as if she is outsode my wordpress web site , how can i do it?
