Creating Custom Forms

I am looking to create a custom form that is not a Profile/Registration/Login form.
Does the UM form builder only generate these types of forms?

Quick outline of what I am looking to achieve:
Site is for a Martial Arts School.
The form I am looking to create is a Grading Application Form. I would like certain fields to pre-populate from the user’s profile.

I don’t want any of the other information recorded on their profile as it won’t be needed.

If I can’t do this with the UM Form builder, is there another form builder that can integrate with UM and pull specific UM User details from their UM profile?
