Creating a reservation form

I am trying to figure out how to create a reservation form for our rental unit in Belize. Want to have fields for Name, Email, Number of people in party ( Adults, Children under 16), dropdown for check-in date and dropdown for check-out date and a submit button. Am new to this so any help would be appreciated.
