    i follow your instructions but after the user submit the form which in the
   pop up it reload again. i enable the “ajax mode” to see if it will fix it but
   than the submit button not working.
 * the coockie set properly so it should show only once per 10 days.
    can you please
   have a look?

 * Have you check this as an admin? Because if you are admin and test mode it’s 
   on you will keep seeing the popup anyway.
 * ps: can’t find our popup on provided page

 * I wasn’t log in as admin and it still pop up. You can see it here.
