Contact form with several options

Dear users

I need to create a contact form where the user may enter credentials (such as name, email, telephone, a comment, etc), but also check boxes concerning which of three different kinds of publications the user would like to receive from me. In other words – a standard “I agree” checkbox will not work – I need to know specifically what boxes the user has checked, i.e. which publication he or she would like to receive from me.

I have looked through several contact form plugins but have yet to find a solution.
I hope you can offer me some insights?
