contact form with attachment

I need a contact form on my website that allows users to attach an image. There is a plugin that does this here: http://www.roge.ru/?p=230.
However, there seems to be a problem because it only sends the images and not the text. In the comments on the plugin page someone seems to have fixed it by changing the mime-boundry, but since I have no clue on what this means I desperately need some help. As far as I know there is no alternative for this plugin, so any help on getting this to work would be greatly appreciated.
