contact form validation error

I am receiving this error no matter what I do to the form:
Validation errors occurred. Please confirm the fields and submit it again.

this is the form:

I use WP 4.3 and have woocommerce activated.
Also, I created a new form and it works.
So… I think something is bad in my code…
Can someone please tell me what could be the error? Thanks

I tested with another form (auto-generated)… apparently that one works, though no email arrives to my inbox.
So I adjusted the code to what is below but I still have the validation error: Your entered code is incorrect. + Validation errors occurred. Please confirm the fields and submit it again.

i fixed it. code was bad.

Now I only have to figure out why on the same hosting plan, from 2 different websites one the form works and sends email. and the other no.
