Contact form integration enabled, non-implicit, but still not showing anything

Hi,
I’ve set up as in https://i.imgur.com/e5uGIiz.png but I still don’t see any checkbox on my contact form. The “text” of the contact page is
[contact-form to='tirilhauan@gmail.com' subject='Melding frå {Name} ({E-mail}) på tirilhauan.net'][contact-field label='Name' type='name' required='1'/][contact-field label='E-mail' type='email' required='1'/][contact-field label='Message' type='textarea' required='1'/][/contact-form] <a href="mailto:tirilhauan@gmail.com">tirilhauan@gmail.com</a> / (+47) 41453399 [mc4wp_form id="9118"](I ended up putting a short, regular form there so there’d at least be something, but that’s not very user-friendly!)
What am I doing wrong? Do I have to enable something else too, or add some field to the contact form?
