I set up a basic form and put up on my site (not the one in my user name).

When I open the form it has stuff already in the fields.

First Name – Admin
Last Name – Easy Contact
Email – has my email adresss prepopulated.

How do I set up the form so that these fields are blank when someone clicks on the “contact” link?

If you log out and access the form, no data will be prepopulated.

The form is grabbing the user data, namely you. If someone else is logged in and accesses the form, it will be their data.

That being said, you can go to the Field and select i.e. First Name. At the very bottom “Registered user options” check “No actions.” Thus, no data will be filled out, even if you are logged in.
