Contact Form 7 Style tempalte activated but not displaying

    I installed Contact Form 7 Style and completed setup. I chose the Monchrome Light template and applied it to the existing contact form in Contact Form 7. However, the original, default form is showing. Not sure what I need to do to get the Monochrome Light template to display.

    Any advice would be greatly appreciated.
