Contact Form 2.1.2 not saving/sending complete form

We have Jetpack running on two different WordPress sites… on one site, it is running 2.1.2, on the other it is running 2.0.4.
On the 2.1.2 version site, the Contact Forms do not store/save the last question of the form on any of three forms we have set up. The last question on each form is set up as a “text area” field, asking for any additional comments. When you enter a response, hit submit, and it shows you your sent response, the last question appears but with no response stored. The emails we receive also do not have the response.

On our site with version 2.0.4 of Jetpack installed, with the same type of contact form set up, it works fine.

I did notice that some of the contact form code that inserts in your page appears to have been slightly different between 2.0.4 and 2.1.2 (use of “true” for required status versus “1” in the latest version, double quotes versus single quotes in latest version).
Any suggestions?
