Contact Form

I am not sure what version I am using. Where do I find this info?
Anyway, I have created a contact page using advice

But when I view the form on my web page is has my name and e mail address?
How can I have this blank for the reader to put their e mail and contact name?
Am I wrong in thinking reader would complete the form and I would get this via blog/website to respond to?
