    Hi @onqmarketing

    Can you run a sync on your lists in our Lists management menu item, and then edit the form in question to confirm it’s still selecting the intended list, and click save on the form?

    It looks like the list presently chosen isn’t being found in the lists from your account, so we may need to re-sync some details.
