 * Is it possible to add a checkbox to the form, asking for consent (and likely 
   linking to our privacy policy. If the checkbox was not checked, the form could
   not be submitted.

 * This would be very helpful for everyone that has to comply with EU privacy laws.
 * On this subject: does your mailchimp integration paid plugin support the[list based express consent workaround](http://metricmarketing.ca/blog/step-by-step-guide-to-getting-express-consent-for-mailchimp-users)?
   That’s another matter that many of EU citizins have to consider.

 * The mailchimp add-on allows you to choose which list to add the user to. in this
   case, you could add them to the I Consent list in your account. The add-on also
   adds a consent checkbox to the end of your quiz/survey. You can customize the
   text for the checkbox as well.
 * You will want to add some explanation including what the content your subscribers
   receive will include, your contact information and a statement that subscribers
   can unsubscribe at any time into the “Message Displayed At End Of Quiz” which
   displays right before the mailchimp consent checkbox.
