Complicated Contact Form

    I need to make a contact form that looks exactly like the pdf form in the URL link. Clients must be able to fill in the form online.
    Is there an existing plugin that I can use/buy

    ATM I am using Ultimate Form Builder Lite. The problem is that I can not make that many columns.

    All suggestions are welcome

    Thank you. I tried a lot of them already. There are so many plugins. Just wanted to know if anybody has experience with a plugin that meets all the requirements.

    Making it “exactly like” the PDF is not a good idea. Websites change width, are viewed on phones, etc. You need to break out from the column stuff to the degree you can.

    That said, I do all my forms using Formidable Pro. It’s very flexible with regard to layout
