    I just received a comment approval email that someone has commented on a post that I did not publish. The link above appears to be a post, but it’s a photo in my media files, and was going to be used for a future post.

    The comment is definitely spam, but I don’t understand how that was published, or how someone could find it! Please help!

    The only time I have ever seen this issue occur is when the post was at some point actually published and then set back to draft or pending. A very good host could track such events for you.

    Are you sure the post was not published? Or are you sure it isn’t a plugin that is the preview public?

    For some reason the ability to add comments to media is enabled by default, and can be done from the xmlrpc route without login credentials, why this is the default configuration escapes me.

    This is an image not a post, as such you dont need to “publish” it. Its automatically published as its being uploaded.

    Meanwhile, you should try the plugin sugested by @rossmitchell
