Comment author and url mismatch

    Randomly for my blog, comments posted by some authors show a different author name or link to a different author’s website.
    Does anyone know what causes this or how to fix it?
    I’m using
    K2 0.9.1
    I’m guessing one of these plugins causes the problem
    wp-cache 2.0.19
    Akismet
    Bad Behavior (turned off, so shouldn’t be the problem)
