Code being outputted upon clicking a Send button?

Hi there,

I have some code in here from a custom made plugin, but I just thought I’d post this to see if anyone has any ideas. Upon submitting a contact form, a bunch of information is popped up which looks like code from the plugin.

Does anybody know why something like this would happen? If so, what is the solution? Thank you!
