Clicking on Placemark Doesn’t Display What’s in Placemark

I’ve encountered a few problems over the last few days…

1) I have put the map on a few different pages and posts… now it’s not showing up on one page, where it used to

2) When I click on a placemark on the map, It used to correctly display the contents of what I had entered for the placemark (which was the address and a link to my review). Now, it’s displaying what’s on the page.
