 * Good afternoon,
 * Thanks for building such a wonderful plugin. It was working great, and still
   is, for the most part. The issue I’ve run into occurs only when my website is
   running in chrome.
 * On the widget in the sidebar, the Last Name and fill in form for Last Name jump
   to the same line, leaving the entire box looking messy. In Chrome, the order
   appears as such:
 * “First Name”
    “First Name Box” “Last Name” “Last Name Box” “City or Town” “City
   or Town Box” “Email Address” “Email Address Box”
 * Is there anything I can do, or that you can help with, to get this sorted out?
 * Many thanks in advance.
