chosen subcategory adding as a parent category on form wordpress issue

hii to all and word press pros please help me i am having a form and i am calling subcategories using ajax when i choose a parent one, but what is happening after filling the form i mean when i click on parent category the child category come and i choose anyone and when submit the form it will add that subcategory next time as a parent category in the same form no idea why its happening please share your idea

then choose any one and fill the form and it will come up when you again posting in parent tab
