Checklist selections won’t appear on submissions or email

Hello @iccmasji,

I am sorry to hear that the selection submissions are not getting through.
Can you create a new test site, just install the forminator plugin, export the form from the present plugin by navigating to your current site wp-admin Dashboard-> Forminator-> Forms-> Beside the form name you should see a gear icon, click on the gear icon and then click not the Export button.

Download the form and then import it to your test site. To import the form, please navigate to your new test site wp-admin Dashboard-> Forminator-> Forms-> Import

To create a test site on your desktop if your server does not permit making test sites, you can use the Local by flywheel tool to make the same.

Add this form to a page and then check if submission works for you. If that does not, please send us the form export here. Please make sure to strip off all the confidential information like the emails from the email notifications, save the form and then send us the form export here by uploading the form to your Google Dropbox. Please make sure you keep the form permissions adjusted so we can download it on our end to test the same. Also, please let us know your site WP version & the PHP Version of the site as that will help us better troubleshoot it.

To change the site permalinks, please navigate to the Permalinks section from your site wp-admin Dashboard-> Settings-> Permalinks section. Here is are the step by step instructions with the screenshots to it too:
