Checkboxes not working on pages containing multiple forms

    I have added forms containing checkboxes in each of three tabs (Homeowner, Contractor and Other). The checkboxes work in the first tab (Homeowner), but not in the remaining forms (Contractor and Other). Fields before and after the checkboxes work fine.

    Hello!

    In a page that has multiple forms the fields ids should be different, otherwise the ids of one form could be conflicting with the ids of the other.
